Create your AZ Complete Health Account.

  1. Go to the AZ Complete Health website (available in the ‘CPR Bookmarks’ button when you’re logged into your CPR Google account) and click ‘Create Account’, in the upper right corner.
  2. Use the following information to create your account:
    1. Tax ID=   421528589
    2. Your first & Last name
    3. Your CPR email address
    4. Password of your choice
  3. Check your email for the verification code sent by noreply@azcompletehealth.com.
    1. Copy this code, then go back to the account registration page, paste it into the ‘Enter Code Here’ field, and click ‘Confirm’.
  4. Complete your “Secret Questions”
  5. In the “Contact Information” fields, enter CPR’s phone and fax numbers:
    1. CPR Phone: 480-804-0326
    2. CPR Fax: 480-804-0083
  6. Submit, and then await your profile activation email, as indicated on the ‘Registration Complete’ page:
    An AzCH-Complete Care Plan provider services specialist will be sending you an email when your profile has been activated. Please allow up to 2 business days for processing. If you do not receive an email within 2 business days, please log in and contact us using secure messaging or call 1-888-747-3627 for additional assistance.
